Job Description

American Traveler is seeking an experienced RN for a Home Health Float position in Massachusetts requiring 1 year of certified home care and case management experience.

Responsibilities

• Work is carried out in various home health locations in the North Region, including Arlington, Everett, Malden, Medford, West Medford, Melrose, and Somerville
• Float RN position covering home visits as needed across assigned territories
• Patient population ranges from infants to geriatric clients requiring clinical services at home
• Schedule is day shift from 8:00 AM to 4:30 PM
• Weekend rotation is required once every 6 weeks
• Holiday and on-call coverage as needed per department rotation
• Uses Epic EMR for electronic documentation
• Responsibilities include clinical assessments, care plan implementation, and patient/family education in accordance with departmental policies
• Position includes admissions and case management duties with thorough electronic documentation and interdisciplinary communications
• Must be willing and able to travel between multiple locations within the assigned region
• Local candidates must reside at least 60 miles from Boston for standard travel rate eligibility
• Public transportation and paid parking options are available; parking fees may apply
• First time travelers are accepted

Requirements

• Active MA RN license required; conditional or pending licenses are not accepted
• Minimum of 1 year of certified home care nursing experience
• Minimum of 1 year of case management experience
• Current AHA certifications required
• Reliable independent transportation for home visits
• Primary source license verification required (NURSYS and BON)
• Flu vaccination required with no religious or medical exemptions
