HumanEdge Travel Healthcare is seeking a local contract nurse RN Home Health for a local contract nursing job in Bronx, New York.

Community Health Nurse – Bronx, NY

Available Schedule 

Tuesday-Saturday shift 

Sunday-Thursday shift.

Monday-Friday shift with 1 weekend every 6 weeks.

The Community Health Nurse will be making 4-5 visits per day. Home care experience is required.

What You’ll Do:

• 
  

  Conduct skilled nursing visits and health assessments

• 
  

  Deliver person-centered care using the full nursing process

• 
  

  Educate members and families on health management

• 
  

  Maintain documentation in accordance with regulations and policies

• 
  

  Collaborate with the care team to support holistic patient needs

Education:

NYS licensed RN

Certifications:

BLS

What We’re Looking For:

• 
  

  NYS Registered Nurse license

• 
  

  Prior experience with community health, home care, or PACE preferred

• 
  

  Strong assessment, communication, and organizational skills

• 
  

  Commitment to culturally sensitive and compassionate care
