We are a Primary Care group who does preventive care to the less fortunate.
What You’ll Do:
Conducting Patient Assessments: Performing physical exams, taking medical histories, and evaluating symptoms to diagnose and manage acute and chronic conditions.
Developing Treatment Plans: Prescribing medications, ordering diagnostic tests, and providing evidence-based treatments for various health conditions.
Providing Preventive Care: Administering vaccinations, conducting screenings, and offering health education on topics like nutrition, exercise, and disease prevention.
Managing Chronic Diseases: Helping patients with conditions like diabetes, hypertension, and asthma by developing long-term care plans and coordinating specialist referrals.
Collaborating with Healthcare Teams: Working alongside physicians, nurses, social workers, and behavioral health professionals to provide holistic, patient-centered care.
